Lesson Release Forms
Kathy Culler Release Form: All riders must submit a completed Lesson Release Form, prior to their first lesson with Kathy Culler.

Running Wind Farm Release Form: Any riders taking lessons with Kathy Culler at Running Wind Farm (RWF) in Midland, NC must also complete and sign the RWF release form.
​
Optional Photo Release Form: All riders are encouraged to complete and submit the photo release form.​
